Application and synthesis of polymyxin B sulfate
Indication
Polymyxin B sulfate, similar to polymyxin E, has inhibitory or bactericidal effects on gram-negative bacteria, such as Escherichia coli, pseudomonas aeruginosa, paraescherichia coli, Klebsiella pneumoniae, Acidophilus, pertussis bacillus and dysentery bacillus. It is mainly used for wound, urinary tract, eye, ear, trachea and other infections caused by pseudomonas aeruginosa and Chemicalbook, and can also be used for sepsis and peritonitis. And resistance to aminoglycoside, third generation cephalosporin and pseudomonas aeruginosa or other sensitive bacteria caused by serious infections, such as bacteremia, endocarditis, pneumonia, post-burn infection, etc.
Usage and dosage
- Intravenous drip: Adults and children with normal renal function 1 day 1.5 ~ 2.5mg per kilogram of body weight (generally no more than 2.5mg per kilogram of body weight), divided into 2 times, 1 time every 12 hours of chemicalbook. For every 50mg of this product, dilute 500ml with 5% glucose solution and drop. Infants with normal kidney function can tolerate a daily dose of 4mg per kilogram of body weight.
- Intramuscular note: Adults and children: 2.5-3mg per kilogram of body weight per day, given in divided doses, once every 4-6 hours. Infant 1 day dose can be used up to 4mg per kilogram of body weight, new Birth can be up to 4.5mg per kilogram of body weight.
- Intrathecal injection (for pseudomonas aeruginosa meningitis) : sodium chloride injection is prepared with 5mg per milliliter of liquid medicine. Adults and children over 2 years old, 5mg daily, after 3 to 4 days, change to every other day, at least 2 weeks, until the cerebrospinal fluid culture negative, the test sugar content is normal. For children under 2 years of age, 2mg once a day for 3 to 4 days (or 2.5mg once every other day), then 2.5mg once every other day until the test is normal.
- The concentration of eye drops is 1-2.5 mg per milliliter.
Drug interaction
Avoid the combination of drugs with greater damage to the kidney, skeletal muscle relaxants, aminoglycoside antibiotics, and anesthetics with obvious muscle relaxation effects (such as enflurane), and should not be used intravenously with quinine, magnesium, etc. Polymyxin B sulfate combined with sulfonamides, rifampicin, semi-synthetic penicillin, etc. for the treatment of severe drug-resistant gram-negative bacteria infection, the effect is better than that of single application.
Matters needing attention
1, pregnant and lactating women, children, more damage to the kidney see severe renal insufficiency patients with caution or reduction.
2, this product is more toxic, poor effect on deep tissue infection, is not the first choice for any infection.
3. Intravenous injection may cause respiratory depression and is generally not used. At least half of the daily dose should be administered intravenously and should not be administered rapidly at once to avoid widespread neuromuscular block.
4. It should not be combined with other drugs with renal toxicity or neuromuscular blocking effect to avoid accidents.
